Enrol online
There’s no need to come into the clinic, you can enrol online - it’s secure and confidential. The online form will guide you and you will need to upload suitable identification
Paper Enrolment
Paper enrolment forms are still available if you're not able to complete the online form. For each person enrolling, simply print and complete: the Enrolment Form, an Adult or Child Medical Questionnaire and email all files to admin@drury.thedoctors.co.nz including a copy of your suitable identification
